Click here to download the Enrollment packet and return to your campus or email the packet to hello@leadingedgepreschool.org. After the packet is returned, we will add you to our system, Procare, and your child can begin at Leading Edge Preschool. All invoices will come from Procare.

A completed packet includes:

AZDHS Emergency Form
Immunization records or Religious Exempt Form
Enrollment Agreement
Desired Program Form
